Abstract

Background: Laparoscopic tubal recanalization (LTR) is a minimally invasive surgical procedure that restores fertility in women following sterilization, offering a cost-effective alternative to assisted reproductive technologies. The laparoscopic threestitch technique, known for its precision and excellent outcomes, has become a widely preferred method for tubal recanalization, and this paper will focus on its role in optimizing surgical success.

Objective: To evaluate fertility outcomes of three-stich technique of laparoscopic tubal recanalization.

Materials and methods: This prospective observational study was conducted over a period of three years from Sept 2020 to Aug 2023 at Radhakrishna Multispecialty Hospital and IVF Centre, Bengaluru. A total of 25 patients, who underwent laparoscopic tubal recanalization using the three-stitch technique were included. All the subjects were followed up for one-year post-surgery to evaluate fertility outcomes.

All patients underwent a comprehensive pre-operative evaluation, including a basic pelvic examination and transvaginal
ultrasound to assess uterine and adnexal health. Partner investigations included semen analysis to ensure normal sperm
parameters. Surgical feasibility was confirmed intraoperatively with particular attention to tubal length and the extent of
damage. Post-operative follow-up included monitoring for tubal patency and natural conception rates, along with secondary
evaluations for tubal-related complications or ectopic pregnancies.

Results: With a mean patient age of 32.4 years and an average operative time of 78 minutes, the technique achieved a
conception rate of 52%. Shorter sterilization-to-recanalization interval (<3 years) was associated with significantly higher
conception rates (82%) compared to longer interval (>5 years, 12.5%).

Conclusion: The three-stitch method, utilizing microsurgical principles, demonstrated superior efficiency, reducing operative
time compared to the conventional four-stitch technique, while maintaining excellent reproductive outcomes. These findings
underscore the importance of patient selection, residual tubal length, and meticulous technique in achieving optimal fertility
restoration through LTR.
