Laparoscopy has become an accepted method of management of ovarian cysts. The use of the laparoscopic approach versus open surgery for the management of giant ovarian cysts is controversial. Conventional treatment for giant ovarian cyst is full midline laparotomy. A laparoscopic approach to giant ovarian cysts, when the cyst's size exceeds the umbilicus, may be difficult due to the risk of cyst rupture and limited working space. However, the advantages due to laparoscopy of lesser morbidity cannot be ignored. Exclusion of malignant conditions has critical importance in the laparoscopic management of these cases.
