Interlocking in diamniotic twins is a rarity. It is a hazardous obstetric complication. Herein, we report one such rare case which was unbooked and attended to our labour room in active labour. The foetus which was delivered vaginally could survive and that delivered abdominally could not be revived due to severe hypoxic injury. The condition of locked twins should be anticipated in twin gestation where first twin is breech and second vertex. Planned and timely intervention can bring favourable foetomaternal outcome.
